Fayette County Board of Education - List of Required Documents
When registering a new student in Robert J. Burch Elementary School you must bring the following list of documents with you to the school office:
Property Deed, or Signed Lease Agreement or Affidavit of Residence
(if living with someone else)
Current Gas or Electric Bill
Certified Birth Certificate
Immunization Record on Georgia Form 3231 (From Dept. of Health)
Eye, Ear, and Dental Record on Georgia Form 330 (From Dept.
of Health)
Social Security Card
Photo ID of Parent/Guardian Registering Student
